Our clinics are certified as Patient Centered Medical Homes and have already or will be undergoing practice re-design focusing on exemplary patientcenteredness and a superior patient experience.

Key Position Essentials:

• Full time providers provide 32 hours/week of clinical care.

• We have a fully integrated Electronic Medical Record through EPIC, as well as pharmacy and care management support.

• Clinical site renovations are ongoing or completed at each location.

• Teaching opportunities with students and residents.

• Successful candidates will be Board Certified/Board Eligible in Family Medicine, with experience practicing the full spectrum of Family Medicine.

• You’ll need to be able to obtain a Colorado Medical License and possess a current DEA Certificate, and successfully complete medical staff credentialing at University of Colorado Hospital.

• Two years of practice experience in ambulatory and Patient Centered Medical Home/Integrated Practice settings is preferred. Individuals with other clinical or practice experience will be considered.

• Additional know-how required includes: a demonstrated ability to establish and maintain effective working relationships at all levels of the organization, the ability to work effectively as part of a team, and experience with teaching students and residents.
